Contract Details:

• Start Date: 11/24/2025
• Contract Type: Permanent
• Shift: Days (Mon–Fri, 8 AM–5 PM)
• Hours/Week: 40
• Pay Rate: $22.39 – $28.09 per hour

Role Overview:

As a Home Care Coordinator, you’ll serve as a vital link between participants, caregivers, and service providers. You’ll conduct home safety evaluations, coordinate delivery of meals, caregiver services, and durable medical equipment (DME), and ensure that care is delivered in a person-centered, compassionate manner. You’ll work closely with the interdisciplinary team to align services with each participant’s goals and preferences.

Responsibilities:

• Coordinate delivery of meals, DME, and caregiver services
• Conduct home safety evaluations during enrollment, address changes, and semi-annual reviews
• Document service delivery and progress in EMR systems
• Monitor quality and timeliness of services from external vendors
• Advocate for participant needs and preferences
• Support nurses with logistics and coordination
• Conduct monthly visits for homebound and RCFE participants
• Deliver supplies and assist with participant surveys
• Participate in IDT meetings and staff training
• Maintain compliance with HIPAA, OSHA, and internal policies

Qualifications:

• License/Certifications:
• CNA certification
• California Department of Public Health Home Health Aide certificate (HHA)
• CPR and First Aid certification from the American Heart Association
• Valid California Driver’s License
• Experience: Minimum 1 year working with frail or elderly populations
• Education: High school diploma or equivalent with required certifications

Preferred Skills:

• Strong communication and coordination abilities
• Familiarity with EMR systems and Microsoft Office tools
• Understanding of HHA scope of practice
• Ability to work independently and within a team
• Compassionate, culturally sensitive approach to care
